Breast Reconstructive Surgery (CPB 0185, reviewed 2026-04-22)
Aetna considers breast reconstructive surgery medically necessary after a medically necessary mastectomy or lumpectomy that results in a significant deformity. Medically necessary procedures include autologous fat grafting, tissue expander use, breast prosthesis implantation, mastopexy, nerve coaptation for improvement of sensation in breast reconstruction for cancer, and flap reconstructions such as DIEP, TRAM, latissimus dorsi, superior gluteal artery perforator and profunda artery perforator flaps. Alloderm, Alloderm-RTU, Cortiva, DermACELL, DermaMatrix, FlexHD, Strattice and SurgiMend are medically necessary acellular dermal matrices, while Artia, the Biodesign Nipple Reconstruction Cylinder, SimpliDerm for breast reconstruction, the body lift perforator flap technique, intraoperative hypothermia in breast reconstruction, venous super-charging and 3D volumetric imaging are considered experimental, investigational or unproven. Aetna considers breast reconstructive surgery cosmetic for all other indications not listed in the policy, and many Aetna plans specify that certain surgeries, such as post-mastectomy breast reconstruction, are not considered cosmetic.
