Collagen Dressings & Collagen-Based Wound Fillers (Surgical Dressings)
Blue Cross Blue Shield of Arizona's new Evidence-Based Criteria for collagen dressings and collagen-based wound fillers (effective 01/01/27) define when these products are medically necessary: a full-thickness wound (e.g., stage 3 or 4), light-to-moderate exudate, or a stalled wound, prescribed by a physician or wound specialist after evaluation, with documented location, type, stage, size, appropriate dressing size, and change frequency if more frequent than standard. Claims for these products (HCPCS A6010, A6011, A6021-A6024) that do not meet these criteria, or that involve heavy exudate, third-degree burns, or active vasculitis, will be considered not medically necessary.
