Occipital Nerve Injections and Ablation (Including Occipital Neuralgia and Headache)
Effective 2026-12-01, the Surest medical policy on occipital nerve injections and ablation removes greater occipital nerve block injections of local anesthetics and/or steroids from both the proven list for pain from head and neck malignancy and the unproven list for occipital neuralgia and headaches (including migraine and cervicogenic headache), and it removes occipital neurectomy from the unproven list. CPT/HCPCS codes 64405, 64570, 64575, 64771, 64999 and A4540 are also removed from the applicable codes list, and the neurectomy definition is deleted. Billing teams should review how these services are scheduled and billed for Surest members starting that date.
