Effective January 1, 2026, Harvard Pilgrim Health Care Commercial plans will deny claims for specific procedure codes unless proper diagnosis codes are also submitted. Claims for procedure codes 64561, 64581, or A4290 require a diagnosis of urinary or fecal incontinence, and ambulatory blood pressure monitoring codes (93784-93790) require elevated blood pressure without hypertension diagnosis.
Blue Shield of California's medical policy 1.01.02 on automated 24-hour ambulatory blood pressure monitoring for diagnosing hypertension has an effective date of November 1, 2026. The policy statement covers this monitoring when it is performed 1 time to differentiate white coat hypertension from true hypertension and when one of two sets of criteria is met. For individuals aged 13 and older, office blood pressure should be SBP greater than or equal to 130 or DBP greater than or equal to 80 mmHg and less than 160/100 mmHg; for individuals younger than 13, office BP should be the lower of at or above the 95th percentile based on gender, age and height per the AAP 2017 pediatric tables, or at or above the adult cutoff; and in both cases the clinical presentation should not require immediate initiation of antihypertensive medication based on the severity of BP elevation or acute hypertensive end-organ injury.
Maryland Medicaid has updated Remote Patient Monitoring (RPM) and Self-Measured Blood Pressure (SMBP) coverage, eliminating prior authorization requirements for RPM and adding new billable SMBP codes 99473 and 99474. Code S9110 is no longer covered for RPM services.
Aetna considers automated ambulatory blood pressure monitoring medically necessary when a member meets one of the listed criteria: at least two elevated office readings taken on separate days to identify 'white coat' hypertension; episodic hypertension with symptoms suggesting an adrenal tumor and repeatedly normal office readings; hypotensive symptoms or syncope thought related to antihypertensive medications; syncope evaluated with a 24-hour Holter monitor; nocturnal angina; resistant hypertension before an invasive investigation for secondary causes; or autosomal dominant polycystic kidney disease in children and young adults 30 years of age or younger. Monitoring for less than 24 hours or more than 3 days is not considered medically necessary, and repeat testing is not generally necessary more often than every 6 months. Aetna considers the use experimental, investigational, or unproven for heart failure, for pregnant women who do not meet the criteria, for diagnosing malignant hypertension, for members with an irregular cardiac rhythm such as atrial fibrillation, for routine monitoring to establish a hypertension diagnosis or evaluate treatment response, and for routine use before continuous positive airway pressure (CPAP) therapy.
Automated 24-hour ambulatory blood pressure monitoring may be considered medically necessary for individuals with elevated office blood pressure when it is performed one time to differentiate between white coat hypertension and true hypertension, the office blood pressure elevation is in the mild-to-moderate range (<180/110 mm Hg) and does not require immediate treatment with medications, and there is an absence of hypertensive end-organ damage on physical examination and laboratory testing. All other uses are considered investigational, including repeated testing in individuals with persistently elevated office blood pressure and monitoring of treatment effectiveness. For pediatric individuals, the policy follows the 2022 American Heart Association guidelines on ambulatory blood pressure monitoring in children and adolescents, including selecting a device with a cuff size appropriate to the child and using pediatric normative data for thresholds.
