Meniere's Disease and Related Disorders: Surgery (CPB 0514, reviewed 2026-07-02)
Aetna considers these surgical interventions medically necessary for chronic refractory Meniere's disease: endolymphatic mastoid shunt, endolymphatic sac decompression or drainage, lateral semi-circular canal plugging, perilymphatic fistula patching, sacculotomy, tympanostomy tube insertion, vestibular nerve decompression, vestibular neurectomy or neurotomy, intra-tympanic corticosteroid injections or perfusions, cochleosacculotomy, intra-tympanic gentamicin, labyrinthectomy, translabyrinthine vestibular neurectomy and vestibulocochlear neurectomy. The policy also covers middle fossa craniotomy and trans-mastoid canal plugging for superior semicircular canal dehiscence when audiometry and computed tomography confirm the diagnosis, and intra-tympanic or trans-tympanic dexamethasone injection for sudden sensorineural hearing loss in members who failed conservative treatment including oral steroids or who have a medical condition that prohibits oral steroids. Aetna considers tenotomy of the stapedius and tensor tympani muscles, triple semi-circular canal plugging, positive pressure therapy for improving outcomes of endolymphatic sac surgery, cochleostomy with neurovascular transplant, intra-tympanic dexamethasone thermo-sensitive gel, and simultaneous labyrinthectomy with cochlear implantation for bilateral Meniere's disease experimental, investigational or unproven because their effectiveness has not been established.
