Balloon Dilation of the Eustachian Tube 7.01.101
Excellus BlueCross BlueShield policy 7.01.101 considers balloon dilation of the Eustachian tube (BDET), unilateral or bilateral, medically appropriate for chronic obstructive Eustachian tube dysfunction in adults 18 years or older who meet all listed criteria. The criteria include an abnormal tympanogram (Type B or C), an abnormal tympanic membrane on examination, other causes of aural fullness ruled out, aural fullness and pressure with hearing loss or otalgia, symptoms lasting three months or longer that are continuous, and no contraindication such as patulous ETD or dehiscence of the carotid artery canal. Failure of medical management of coexisting conditions is part of the treatment history where applicable. BDET is investigational as a repeat procedure, when performed with tympanoplasty or other middle ear surgery, for transtympanic BDET, for ages 17 and younger, and for extrinsic causes of ETD. BDET can only be performed with an FDA-approved balloon dilation device or system. The policy notes that CMS does not address balloon dilation of the Eustachian tube in national or regional Medicare coverage determinations. The current effective date is July 16, 2026, and the annual review states policy intent is unchanged.
