Hyperhidrosis (CPB 0504, reviewed 2026-08-11)
Aetna considers surgical treatments for primary hyperhidrosis of the axillae, palms or soles medically necessary only for members who are unresponsive or unable to tolerate oral pharmacotherapy, have significant disruption of professional and/or social life from excessive sweating, have topical aluminum chloride or other extra-strength antiperspirants that are ineffective or cause a severe rash, and have failed to adequately respond to iontophoresis. A trial of botulinum toxin can be substituted for iontophoresis in persons with predominantly axillary hyperhidrosis. Laser treatment, microwave therapy, miraDry, pulsed radiofrequency, radiofrequency micro-needling, photodynamic therapy, liposuction-curettage and ultrasound are listed as experimental, investigational or unproven for hyperhidrosis.
