By December 31, 2025: Billing team must review and understand Policy 00.10.39r requirements for office-based services billed from facility-campus locations. Identify all practice locations that operate within or on a facility campus and audit current billing practices against the new policy guidelines. Update billing software, claim submission templates, and provider documentation requirements as needed to ensure compliance with facility-campus billing rules. Training on the new policy must be completed for all billing and clinical staff who handle claims from these locations. Failure to comply may result in claim denials or recoupments.
