(2) For CPT 47370, 47380, 47382 (hepatic procedures): Verify colorectal metastases or neuroendocrine tumors, surgical candidacy status, tumor size/count limits (≤5cm single or ≤3cm x3 multiple), and eradication of all intrahepatic lesions. (3) For CPT 50592 (renal): Confirm Stage T1a ≤3cm or T1b with non-surgical candidacy. (4) For CPT 60699 (thyroid): Verify malignant metastases/recurrent disease with RAI non-amenability or medullary subtype. (5) For CPT 37243 (RFA+TACE combination): Require solitary 3-5cm HCC, non-resectable, localized disease only. (6) DENY all claims for CPT 19499 (breast) and prostate RFA, and all spinal RFA procedures regardless of documentation. Providers must submit supporting clinical documentation with claims; billing staff must flag missing criteria before submission. Update encounter templates to prompt providers for required documentation fields. Non-compliant claims will be denied by payer.
