Summary
Aetna's CPB 0822 (effective 2025-12-09) establishes coverage for internal fixation of rib fractures ONLY in severe flail chest cases failing to wean from ventilator or when thoracotomy is required for other reasons. All other indications—including bio-absorbable plates and minimally invasive plate osteosynthesis—are deemed experimental/investigational and non-covered. Billing teams must deny claims for rib fixation that don't meet these specific criteria.

By December 9, 2025: (1) Billing team must update claim submission software to require medical necessity documentation linking rib fixation (CPT 21811-21813) ONLY to flail chest (ICD-10 S22.5xx) with documented ventilator weaning failure or concurrent thoracotomy. (2) Configure system to AUTO-DENY claims for CPT 21811-21813 when paired with other rib fracture diagnoses (S22.31x-S22.49x) or when bio-absorbable plates or minimally invasive techniques are documented. (3) Update encounter forms and provider templates to clearly flag that Aetna covers rib fixation only for severe flail chest in specific clinical scenarios. (4) Train billing staff to request operative reports documenting ventilator status and clinical justification before submission. (5) Establish prior authorization requirement: submit operative note and ventilator weaning history with all rib fixation claims to Aetna. Claims submitted without this documentation or for non-covered indications will be denied as experimental/investigational.
