By 2025-12-30, billing team must: (1) Update billing system to enforce prior authorization requirements for CPT 61343 to verify tonsillar displacement measurements (≥5mm or ≥3mm in pediatric cases) and neural compression on imaging; (2) Create encounter form checklist requiring providers to document: radiographic findings of tonsillar displacement with measurements, evidence of neural structure crowding/compression at foramen magnum, presence/absence of syrinx, and for isolated headaches/neck pain—neurologist documentation of conservative treatment failure; (3) For sleep apnea or swallowing difficulty cases, ensure sleep study and/or video swallow evaluation results are attached to claims; (4) Configure system to DENY claims for experimental procedures: myo-dural bridge protection/strengthening, intradural dissection without dural closure, extended C2 laminectomy, C1 tubercle removal, and AI prediction tools—add denial message referencing experimental status; (5) Train providers and billing staff on stricter documentation requirements; (6) Contact neurosurgery and neurology practices to confirm they understand documentation expectations. Claims lacking required documentation or using experimental techniques will be denied.
