Summary
Aetna updated its Esophageal Doppler Monitoring (EDM) policy clarifying that EDM is medically necessary ONLY for intra-operative fluid optimization or monitoring ventilated ICU patients. All other indications are considered experimental/investigational and will not be covered. Billing teams must ensure claims for EDM outside these two approved scenarios are not submitted or will face denials.
Action Required
By December 2, 2025: Billing and clinical teams must implement the following controls: (1) Billing team: Update billing system to flag any EDM claims that do NOT include either ICD-10 code J95.850 (mechanical complication of respirator - for ventilated ICU patients) or Z99.11/Z99.12 (dependence on respirator) as non-covered; (2) Providers: Update operative and ICU documentation templates to clearly document which of the two approved indications applies when EDM is used; (3) Prior authorization: Implement pre-service review for all EDM requests to verify the patient meets one of the two medical necessity criteria before proceeding; (4) Denials management: Prepare to handle and appeal denials for EDM claims that lack proper ICD-10 coding or documentation of approved indication. Claims submitted without evidence of intra-operative fluid optimization OR ventilated ICU status will be denied as investigational/unproven.
