By 2026-08-03: Billing team must implement the following: (1) Update billing system to require precertification for ALL canakinumab claims directed to Aetna at (866) 752-7021 or fax (888) 267-3277 before processing; (2) Configure system to flag claims if TB screening documentation (TST or IGRA within 12 months) is missing and deny claims without this documentation; (3) Update claim validation rules to reject any claim showing concomitant use of canakinumab with other biologic drugs or targeted synthetic drugs (e.g., adalimumab, anakinra, etanercept, infliximab, rilonacept, tocilizumab, tofacitinib); (4) Create encounter form checklist for providers to document medical necessity criteria specific to each indication (e.g., PGA score ≥2 or CRP >10 mg/L for TRAPS/HIDS/MKD/FMF; three gout flares in 12 months plus inadequate response to NSAIDs/colchicine/corticosteroids); (5) Ensure billing team verifies prescriber specialty matches approved specialties (rheumatology or immunology for CAPS/TRAPS/HIDS/MKD/FMF; rheumatology for sJIA/AOSD/gout/pseudogout); (6) Configure system to accept only approved indications and deny claims for experimental indications (e.g., COVID-19, Kawasaki disease, heart failure, atopic dermatitis). Consequence: Claims submitted without precertification, TB documentation, or proper medical necessity will be denied by Aetna.
