By April 17, 2025: Billing team must implement prior authorization workflow for LINX device implantation procedures. Providers must submit: (1) Documentation of chronic symptomatic GERD confirmed by endoscopy, fluoroscopy, or ambulatory pH testing; (2) Evidence of inadequate PPI control for ≥6 months or medical contraindication to PPI; (3) Confirmation of hiatal hernia repair if >3cm; (4) Verification of provider credentials and procedure-specific training; (5) Attestation that patient has none of the listed contraindications (titanium/metal allergies, need for MRI >1.5T, BMI >35, esophageal malignancy, major motility disorders, electrical implants, varices, strictures, scleroderma). Update prior auth request forms to include mandatory fields for each criterion. Flag claims for denial if any of the 9 contraindications are present or documentation is incomplete. Front desk staff must inform referring providers that shared decision-making documentation (including procedure explanation, risks/benefits, condition management, and alternatives) must be completed and retained prior to surgery. Claims submitted without complete prior authorization will be denied.
