Effective immediately (as of June 23, 2026): Billing team must verify all Anthem MyCare Ohio home health claims for skilled state plan-covered services using HIPPS codes with REV code 022 and confirm whether the practice has an established per diem reimbursement rate. For providers WITHOUT a contracted per diem rate, update billing software to automatically generate and submit claims on UB-04 claim forms with bill type 329. Train billing staff to identify claims requiring bill type 329 before submission.
