Action Required
By January 1, 2026: Billing team must update system to require prior authorization for new CPT codes 77437, 77438, and 77439 through EviCore (online or 888-693-3211). Remove deleted CPT code 77014 from billing system. Update encounter forms to alert providers that these radiation oncology codes need prior auth. Claims without prior authorization will be denied.
