Medication Waste (Revised)

Humana revised its Medication Waste guidance policy (effective 12/17/2025) clarifying when single-dose vial waste can be billed with the JW modifier. Key requirement: medication administered and wasted must be billed on the same claim with detailed documentation (date, time, amount, reason), and waste cannot be billed for multi-dose vials. Any discrepancies between billed and documented amounts will result in denials unless wastage is clearly documented.
