Summary

Blue KC is implementing three major policy changes effective between October 1, 2026 and January 1, 2027: (1) enhanced enforcement of drug waste billing with JW/JZ modifiers requiring detailed documentation and waste minimization, (2) three new codes requiring prior authorization effective November 1, 2026, and (3) revised inpatient readmission policies that combine related readmissions within 15 days into a single payment. Billing teams must update claim submission processes, prior authorization workflows, and documentation requirements across multiple lines of business.

Action Required

Before Jan 1, 2027
REQUIREMENTS: 1. BY NOVEMBER 1, 2026: Billing team must add three codes to prior authorization list in billing system: L0999 (spinal orthosis addition), E0676 (intermittent limb compression device), and L5999 (lower extremity prosthesis, not otherwise specified). Update claim submission workflows to require prior authorization requests for these codes on Commercial and ACA lines of business before claim submission. Flag claims missing prior auth authorization for denial prevention. 2. BY NOVEMBER 1, 2026: Billing team and provider staff must update hospital billing procedures for inpatient readmission policy (POL-PP-239). Train revenue cycle staff to identify readmissions within 15 calendar days of prior discharge to same health system. Combine qualifying readmissions into single payment per contractual agreement rather than billing separately. Update system logic to flag potential readmission scenarios and route for manual review. 3. BY OCTOBER 1, 2026: Billing team must implement Revenue Code 0710 (Recovery Room) enforcement for outpatient surgical procedures. Require that revenue code 0710 be billed ONLY on day of surgery/procedure AND in conjunction with valid anesthesia revenue code 037X. Configure billing system to deny claims with 0710 billed without 037X. Update facility claim submission instructions. 4. BY JANUARY 1, 2027: Billing team and clinical providers must implement drug waste policy (POL-PP-234) enforcement for single-dose vials across Commercial, ACA QHP, Small Group, ACA JAA, FEP, and Medicare Advantage lines of business. SPECIFIC ACTIONS: - Update billing system to require JW (waste) or JZ (zero waste) modifier selection on all single-dose drug claims - Require providers to document in medical record: amount administered, date, time, amount discarded, reason for waste - Update claim validation rules: when drug available in multiple vial sizes (e.g., 20-unit and 100-unit vials), require providers to select minimum vial combination to minimize waste (use two 20-unit vials instead of one 100-unit vial if member needs only 40 units) - Configure system to report reimbursable drug/biologic on one line with units administered, and wasted portion on separate line with JW modifier - Configure system to enforce: JZ modifier required when zero waste from single-dose vial; JW modifier NOT permitted when actual dose is less than billing unit (use JZ modifier instead) - Configure system to deny reimbursement for contaminated, expired, or discarded (spillage/damage/breakage) drug doses from single-dose vials - Train billing staff that HCPCS code units are based on code descriptor (e.g., J2506 pegfilgrastim 0.5 mg = 1 unit) not actual mg administered - NOTE: This policy does NOT apply to Multi-Dose Vials (MDV) - Consequences: Claims not compliant with modifier requirements and waste documentation will be denied.
