Providers must begin transitioning maternity billing practices Previously due September 1, 2026 (date passed; verify current applicability), for deliveries on or after January 1, 2027.
Action Required
REQUIREMENTS:
Previously due September 1, 2026 (date passed; verify current applicability): Billing team should begin transitioning maternity billing processes to individual claim submissions (E/M codes for prenatal visits) for patients expected to deliver on or after January 1, 2027, moving away from global maternity billing codes.
By January 1, 2027: (1) Billing team must implement electronic prior authorization (ePA) workflows for all lines of business (Commercial, ACA QHP, Small Group ACA JAA, FEP, Medicare Advantage, and Dental Blue). Providers with FHIR-capable EHRs should contact their Blue KC Provider Account Executive to enable ePA features. (2) Billing team must enforce enhanced drug waste policy (POL-PP-234) requiring JW and JZ modifier compliance: report drug/biologic HCPCS codes with units administered on one claim line; report wasted units with JW modifier on a separate line of the same claim; append JZ modifier when zero waste occurs from a single-dose vial; ensure documentation in medical records includes amount administered, date, time, amount discarded, and reason for waste. (3) Billing team must calculate drug dosages to minimize waste by selecting appropriate vial size combinations (e.g., use two 20-unit vials instead of one 100-unit vial when only 40 units are needed). (4) Providers must comply with all maternity coding changes and submit individual claims throughout pregnancy rather than bundled global claims. Failure to implement ePA workflows will impair prior authorization turnaround times and increase administrative burden. Failure to comply with drug waste modifier requirements and documentation standards will result in claim denials. (5) Be prepared for post-payment clinical claim validation reviews beginning November 1, 2026, conducted by Cotiviti (formerly Apixio/Machinify); respond to medical record requests via U.S. mail to avoid claim denials.
Already underway (November 1, 2026): Medical billing team should monitor for post-payment clinical claim validation (CCV) requests from Cotiviti and provide requested medical records promptly via U.S. mail to support accurate billing and claim validation reviews.
