Summary
Aetna's updated Prostate Cancer Screening policy (CPB 0521, reviewed 2026-08-28) clarifies coverage for PSA screening and DRE as preventive services for average-risk men 45+ and high-risk men 40+, while explicitly designating 20+ advanced biomarker tests (including PCA3, Stockholm3, four-kallikrein panel, and genetic markers) as experimental/investigational and therefore not covered. Billing teams must ensure claims for covered PSA codes (84152, 84153, 84154) and HCPCS screening codes (G0102, G0103) are submitted only when selection criteria are met, and must deny or reject claims for experimental tests listed in the policy.
Action Required
IMMEDIATELY: Billing team must update claim submission rules in billing software to:
1. ALLOW claims for PSA screening (CPT 84152, 84153, 84154 and HCPCS G0102, G0103) ONLY when patient meets criteria: age 45+ (average-risk), age 40+ (high-risk: African-American or family history), OR age 75+ with documented life expectancy ≥10 years, OR any age with signs/symptoms of prostate cancer.
2. BLOCK or AUTO-DENY all claims for the following experimental/investigational codes: CPT 0021U, 0133U, 0433U, 0495U, 81210, 81313, 81539, 83520, 84255, and molecular cytogenics codes (88271-88275, 88291).
3. Modify claim submission instructions to providers, noting that experimental tests (including PCA3, Stockholm3, four-kallikrein panel, BRAF mutations, genetic-based screening, and hK2) will NOT be covered regardless of medical justification.
4. Create reject reason codes mapping experimental codes to non-coverage rationale for member/provider communication.
FAILURE TO UPDATE: Claims for experimental codes will be submitted and denied post-adjudication, resulting in unnecessary claim rework, member complaints, and potential account reconciliation issues.
