Summary
Washington State Health Care Authority released an updated sterilization consent form (HHS-687) effective immediately. The new form must be used for all initial sterilization consents, but providers can continue using the old form (expires April 30, 2022) for claims through February 28, 2023 if initial consent was already obtained.
Action Required
Immediately: Download the updated HHS-687 sterilization consent form from HCA's Provider billing guides webpage under the Sterilization dropdown. Clinical staff must use the new form for all initial sterilization procedure consents. Continue processing claims with old forms (April 30, 2022 expiration) through February 28, 2023 only if initial consent was already completed.
