Action Required

Action needed
By April 1, 2019: Billing team must update all claim submission processes to include provider taxonomy codes. For paper CMS-1500 claims: place taxonomy code in box 33b with ZZ qualifier for billing level, in shaded portion of box 24J for rendering provider, and ZZ qualifier in box 24I. For UB-04 claims: include billing taxonomy in box 57. For electronic claims: update EDI software to include PRV segments with taxonomy codes in appropriate loops (2000A, 2310A, 2310B, 2420A for 837P and 837I). Claims submitted without required taxonomy codes will likely be rejected or denied.
