Immediately: Billing team must ensure all claim retractions, corrections, and medical record submissions use the Physician/Provider Claim Adjustment Request Form and are mailed to the claims department address on the form. Use the Physician/Provider Appeal Request Form only for true appeals and send to GAU. For timely filing denials due to other plan retractions within 180 days, submit the Claim Adjustment Request Form with copy of other plan's retracted RA showing complete retraction and adjudication date.
