BCBS Rhode Island has updated their appeals request form and claims adjustment form. All provider offices must immediately start using the new forms available on bcbsri.com, and remember that claim adjustments should be mailed to the claims department, not the Grievance and Appeals Unit.

Immediately: All staff must download and begin using the new Appeals Request Form and Claim Adjustment Form from bcbsri.com under provider forms. Update office procedures to ensure claim adjustments are mailed to the claims department address (found on the form), not sent to the Grievance and Appeals Unit. Replace all old forms with the new versions.
