By May 1, 2021: All staff submitting medical records to BCBSRI must attach one of the approved cover sheet forms (Claim Adjustment form, Appeal form, Medical Records form, DME Unlisted Code form, Unlisted Procedure Claim form Professional, Unlisted Procedure Claim form Facility, or Response letter). Download required forms from bcbsri.com > Providers > Forms. Records submitted without proper cover sheets will be returned and delay processing.
