Summary
Effective June 1, 2021, BCBSRI requires modifiers KS (non-insulin dependence) and KX (insulin dependence) for all glucose monitoring claims reimbursement for non-OneTouch brand supplies. Unit limits are also being added to test strips (A4253) and lancets (A4259).
Action Required
By June 1, 2021: Billing team must update system to require modifier KS for non-insulin dependent patients or modifier KX for insulin dependent patients on all glucose monitoring supply claims using codes A4253 and A4259. Update encounter forms to capture insulin dependence status. Review unit limits for test strips and lancets in medical policy. Claims without proper modifiers will not be reimbursed.
