Summary

BCBSRI now requires a valid cover sheet (such as Claim Adjustment form, Appeal form, Medical Records form, DME Unlisted Code form, or Unlisted Procedure Claim forms) on top of all medical records submissions. Starting May 1, 2021, medical records without proper cover sheets will be returned by BCBSRI's Medical Records team.

Immediately: All staff submitting medical records to BCBSRI must attach one of the approved cover sheets (Claim Adjustment form, Appeal form, Medical Records form, DME Unlisted Code form, or Unlisted Procedure Claim forms) to the top of every submission. Download required forms from bcbsri.com > Providers > Forms. Train billing team on new requirement as submissions without cover sheets will be returned starting May 1, 2021.
