Summary

BCBSRI now requires a valid cover sheet on top of all medical records submissions starting May 1, 2021. Medical records submitted without proper cover sheets will be returned by the Medical Records team.

Immediately: All staff submitting medical records to BCBSRI must attach one of the approved cover sheets (Claim Adjustment form, Appeal form, Medical Records form, DME Unlisted Code form, Unlisted Procedure Claim form, Professional Unlisted Procedure Claim form, Facility Response letter) on top of medical records. Download required forms from BCBSRI provider portal under forms section. Records without cover sheets will be returned and delay processing.
