Update lactation consultation billing to list mother as patient only (not infant). For STI testing, bill individual CPT codes as rendered - system will auto-pay highest comprehensive code. Update hearing aid coverage limits to $1,750 per aid. Add modifier QL for ambulance billing codes A0428-A0431 effective November 1, 2025. Claims not following new guidelines will be denied or not separately reimbursed.
