Summary
Effective June 1, 2026, Anthem Blue Cross and Blue Shield (Ohio) is aligning commercial claim adjudication with Medicare standards for single-dose vial administration. Billing teams must now submit two separate claim lines when drug waste occurs: one showing quantity administered (no modifier) and a second line with the JW modifier for discarded amounts. When there is no waste, submit one line with the JZ modifier indicating full dosage was used.
