Blue Cross Blue Shield of Wyoming is offering prior authorization training and implementing new coding requirements effective 11/1 including mandatory origin/destination modifiers for ambulance services and LT/RT modifiers for prosthetics/orthotics.
Action Required
By November 1st: Billing team must update system to require QM or QN modifiers on all ambulance services with origin/destination modifiers. Add LT or RT modifiers to all prosthesis and orthosis claims. Stop using revenue code 0540 for ambulance services as these will deny.
