Set Category II codes to bill at $0.00 or $0.01 charge. Update ambulance service rates with 2.6% increase for ground transport and 3-4% increases for in-network air transport. Switch all authorization submissions to Availity portal only. Claims for CPT 83036 without Category II codes will be denied and require resubmission.
