Summary
Starting July 3, 2026, Blue Cross Vermont will require National Drug Code (NDC), Unit of Measure, and Quantity reporting for all drug services claims in addition to CPT/HCPCS codes. Claims missing this information will be denied and cannot be balanced billed to patients.
Action Required
By July 3, 2026: Billing team must update all billing systems and claim submission processes to include NDC, Unit of Measure, and Quantity for all drug services on professional claims (837P/CMS 1500) and outpatient facility claims (837I/UB 04). Update staff training and encounter forms to capture this required information. Review Section 6.7 of the Blue Cross Vermont Provider Handbook for specific requirements. Failure to include NDC, UoM, and Quantity will result in automatic claim denials.
