Summary
Blue Cross Vermont will begin enforcing National Drug Code (NDC), Unit of Measure, and Quantity requirements for drug service claims processed July 3, 2026 and forward. Claims missing these required elements will be denied and members cannot be held liable for payment.
Action Required
By July 3, 2026: Billing team must update all claim submission processes to include NDC, Unit of Measure (UoM), and Quantity for all drug services on professional claims (837P/CMS 1500) and outpatient/ASC/home infusion claims (837I/UB 04). Update billing software validation rules to require these fields in addition to existing CPT/HCPCS codes. Review current drug service claims to ensure compliance. Claims without NDC, UoM, and Quantity will be automatically denied starting July 3, 2026.
