[New York] QHC compliance: attestation due

By June 30, 2026: If your practice is part of or affiliated with a hospital with more than 50 beds in New York, ensure that hospital completes and submits the Patient Safety Attestation Form to their Anthem provider network manager/contractor. The form requires attestation that the hospital uses a patient safety evaluation system, collects/reports data to patient safety organizations (PSOs), implements person-centered discharge mechanisms, or has implemented an evidence-based quality improvement initiative. Contact your hospital's compliance or network contracting department to coordinate submission. Failure to submit by the deadline may result in contract suspension or termination for the hospital.
