Summary
Indiana Medicaid (IHCP) updated their Preferred Diabetes Supply List effective March 5, 2020, moving three Roche blood glucose monitors (Accu-Chek Aviva, Accu-Chek Nano SmartView, and Accu-Chek Aviva Plus) to non-preferred status due to discontinued manufacturing. The corresponding test strips remain preferred and available for existing users.

Effective March 5, 2020: Billing team must update prior authorization requirements for Accu-Chek Aviva, Accu-Chek Nano SmartView, and Accu-Chek Aviva Plus blood glucose monitors as they moved to non-preferred status for Indiana Medicaid claims. Update encounter forms to inform providers of preferred alternatives (FreeStyle meters, Accu-Chek Guide, TRUE METRIX). Test strips for these meters remain preferred and do not require changes.
