Summary
The Indiana Health Coverage Programs (IHCP) has changed the telephone number for fee-for-service prior authorization requests from the previous CMCS number to 1-800-457-4584, option 7. The fax number and mailing address remain the same, but all phone-based prior authorization requests must now use the new number.

Immediately: All staff handling prior authorizations must update contact information to use the new telephone number 1-800-457-4584, option 7 for IHCP fee-for-service prior authorization requests. Update staff reference materials, phone lists, and any automated dialing systems. Continue using existing fax number 1-800-689-2759 and mailing address.
