Indiana Medicaid (IHCP) will begin covering donor milk bank services starting November 1, 2022. Accredited donor milk banks can now enroll as providers and bill HCPCS code T2101 for human breast milk processing at $5.48 per ounce, but prior authorization is required for up to 5,000 ounces.

By November 1, 2022: If practice works with donor milk banks, billing team must update system to include HCPCS T2101 at $5.48 per ounce rate for Indiana Medicaid. Ensure prior authorization is obtained before billing - up to 5,000 ounces can be authorized. Bill on professional claims (CMS-1500) only. This affects very few practices as most don't work directly with donor milk banks.
