IHCP (Indiana Medicaid) has made U1 and U2 modifiers optional for ALS and BLS ambulance mileage billing (code A0425), effective immediately and retroactive to July 1, 2023. Both ALS and BLS mileage now have the same reimbursement rate of $8.71 per mile regardless of modifier use.

Immediately: Billing team should update ambulance billing procedures to make U1 and U2 modifiers optional when billing A0425 for ALS/BLS mileage for Indiana Medicaid patients. Continue using required modifiers U3 (CAS), U5 (NAS), and SE (MRT). Resubmit any previously denied claims for dates of service July 1, 2023 and later that were denied due to missing U1/U2 modifiers.
