Before April 8, 2024: Update sterilization consent form procedures to allow call-in interpreter attestations. Billing staff must ensure that for any sterilization claims with DOS on or after April 8, 2024, if a language line interpreter was used, a faxed attestation statement with handwritten signature is attached to the HHS-687 or HHS-687-1 consent form. The interpreter's translation date must match the member's consent signature date. Claims may be denied without proper interpreter documentation.
