Summary
Indiana Health Coverage Programs (IHCP) issued a recall notice for specific Accu-Chek Guide Kits that display blood glucose results in wrong units (mmol/L instead of mg/dL), which could lead to dangerous treatment decisions. Providers who dispensed these meters to IHCP members must immediately notify affected patients to stop using the devices and arrange replacements.
Action Required
Immediately: Clinical staff must review patient records to identify IHCP members who received Accu-Chek Guide Care Kits (NDC 6570272910) with lot numbers 207482, 207519, 207526, 207545, or 207454. Contact these patients immediately to verify meter serial numbers against the recall list and instruct them to stop using affected devices. Coordinate replacement meters through appropriate channels (Optum Rx for FFS members at 855-577-6317, or patient's MCE for managed care). Document all patient contacts and meter replacements in medical records.
