Immediately: HCBS providers for PathWays, TBI, or Health & Wellness waivers must create accounts in the OMPP HCBS Certification Portal at in.gov/fssa/ompp if not already done. Monitor email for renewal notifications (sent 3 months prior to due date). When renewal notice received: log into portal, navigate to Applications > Apply for New Application > Certification Renewal, submit all required documents including background checks, TB tests, licensing, and driver's licenses for requested employees within 5 business days. Failure to submit complete compliance reviews will result in corrective action and potential decertification.
