Summary
Indiana Medicaid (IHCP) identified and corrected a claim processing error that incorrectly denied professional and outpatient claims for HCPCS code J2777 (faricimab injection) with dates of service on or after August 26, 2024. Affected claims will be automatically reprocessed or mass adjusted beginning April 9, 2025, and providers can resubmit claims within 180 days of the bulletin publication date for timely filing.
Action Required
By September 2, 2025: Review remittance advices beginning April 9, 2025, for automatically reprocessed claims with ICNs beginning with 52 or 80. If any J2777 claims from August 26, 2024 forward were not automatically adjusted, resubmit them within 180 days of March 6, 2025, including a copy of the first page of bulletin BT202531 to satisfy timely filing requirements.
