Summary
Indiana Medicaid (IHCP) discovered an error causing incorrect denials for COVID-19 vaccine claims using CPT code 91322 for Hoosier Care Connect members from September 2023 through February 2025. The system has been corrected and affected claims will be automatically reprocessed starting April 16, 2025.
Action Required
By September 9, 2025: Monitor remittance advices starting April 16, 2025 for automatically reprocessed claims with ICN/Claim IDs beginning with 52 or 80. For any additional retroactive claims, billing team can submit or resubmit within 180 days of March 13, 2025, and must include a copy of this bulletin when submitting claims beyond standard filing limits.
