Summary
Independence Blue Cross added 12 new HCPCS codes (Q4368, Q4370-Q4373, Q4375-Q4380, Q4382) to the MA11.015ad bioengineered skin substitutes policy effective 07/01/2025. These codes are now designated as medically necessary for wound care treatment, expanding the list of covered skin substitute products for lower extremity chronic non-healing wounds, burns, and post-surgical use.
Action Required
By June 30, 2025: Billing team must update billing system to recognize and process HCPCS codes Q4368, Q4370, Q4371, Q4372, Q4373, Q4375, Q4376, Q4377, Q4378, Q4379, Q4380, and Q4382 as medically necessary for Independence Blue Cross Medicare Advantage plans. Add these codes to fee schedules and prior authorization rules. Update charge capture systems, encounter forms, and provider education materials to include the new codes. Effective July 1, 2025, claims submitted with these codes must reference policy MA11.015ah (which supersedes MA11.015ad). Verify that all coding edits permit bundling or unbundling of these codes per MA11.015ah guidelines. Claims submitted with these codes before system updates are complete may be denied or delayed. Coordinate with wound care providers and ambulatory surgery centers that treat burns, chronic wounds, and perform post-Mohs or post-surgical procedures.
