By June 30, 2025: Billing team and clinical documentation staff must update internal workflows to recognize knee instability as a covered indication for knee orthoses (L1810, L1812, L1820, L1821, L1830, L1831, L1832, L1833, L1836, L1843, L1845, L1850, L1851, L1852) under Independence Blue Cross Medicare Advantage plans. Update prior authorization templates and claim submission guidelines to require documentation of objective knee instability with physical examination findings (e.g., varus/valgus instability, anterior/posterior drawer test results). Train providers and clinical staff that knee instability claims now require specific joint laxity test documentation. Update EMR/billing system encounter forms to include checkboxes for objective instability findings. After July 1, 2025, claims for these codes citing only knee instability without objective examination documentation will be denied.
