Summary

Independence Blue Cross updated MA08.083k to MA08.083n effective October 1, 2025, adding three inflammatory breast cancer ICD-10 codes (C50.A0, C50.A1, C50.A2) as medically necessary for leuprolide acetate 3.75mg (J1950) and 7.5mg (J9217) formulations, but explicitly NOT medically necessary for Fensolvi (J1951) and Camcevi (J1952). This creates differential coverage based on diagnosis and product selection.

Action Required

Action needed
By September 30, 2025: Billing team must update billing system logic to implement differential coverage rules for leuprolide products based on diagnosis coding. Specifically: (1) For inflammatory breast cancer diagnoses C50.A0, C50.A1, or C50.A2, restrict claims to J1950 and J9217 only—deny or route to manual review if billed with J1951 (Fensolvi) or J1952 (Camcevi); (2) Ensure all other leuprolide uses remain subject to existing preferred/nonpreferred product rules; (3) Update claim adjudication rules in billing software to flag mismatches between diagnosis code and HCPCS code for this indication. Providers should verify diagnosis coding accuracy before submission. Failure to implement will result in claim denials for Fensolvi and Camcevi claims with inflammatory breast cancer diagnoses.
