Action Required
By October 20, 2025: (1) Billing team must update system documentation to reference MA11.002q instead of MA11.002p for all new hematopoietic stem cell transplantation claims. (2) Update authorization and billing workflows to recognize newly added mobilizers, including motixafortide (J2277) for autologous HSCT in multiple myeloma patients. (3) Remove any references to the superseded MA11.002p policy from billing system, encounter forms, and authorization templates. (4) Verify that all seven filgrastim variants and related mobilizer codes (J1442, J1447, J2277, J2506, J2562, J2820, Q5101, C9173) are active in billing software. (5) Providers and prior authorization staff should be notified of policy location change and new cross-reference structure. Failure to update may result in claim delays or denials if outdated policy references are used during authorization.
