Summary
Independence Blue Cross MA05.044v DME policy has been updated effective 11/10/2025 to add nine HCPCS codes (A4212, A4337, A4461, A4860, A4932, A6228, A6229, A6230, A6413) to the non-covered items list in Attachment B because they do not meet Medicare's definition of DME. Billing teams must stop reimbursing claims for these codes and update system edits to deny or flag submissions containing these codes.
Action Required
By 11/10/2025: Billing team must update billing system to classify HCPCS codes A4212, A4337, A4461, A4860, A4932, A6228, A6229, A6230, and A6413 as non-covered under Independence Blue Cross Medicare Advantage DME policy MA05.044v. Configure system to deny or auto-reject all claims containing these codes. Update internal DME coverage reference documentation and communicate code list to all providers submitting DME claims. Notify DME suppliers and providers that claims for these items will be denied as non-DME supplies effective 11/10/2025. Failure to implement these changes will result in improper reimbursement and potential overpayment recovery from the practice.
