Summary
Independence Blue Cross updated MA00.019k effective 01/01/2026 to reflect annual CPT/HCPCS code changes for radiologic guidance procedures. Three new codes (33882, 33883, 33886) were added to the covered code list, while eight codes (75842, 75956, 75957, 75958, 75959, 77014, G6001, G6002) were deleted. Billing teams must update their systems to reflect these code changes to ensure accurate claim processing and avoid denials.
Action Required
By 01/01/2026: Billing team must immediately update billing system and encoder software to implement the following changes: (1) ADD CPT codes 33882, 33883, and 33886 to the covered radiologic guidance procedure list in Attachment A; (2) REMOVE CPT codes 75842, 75956, 75957, 75958, 75959, 77014 and HCPCS codes G6001 and G6002 from the covered code list; (3) Update claim scrubbing rules and billing edits to reflect new covered/non-covered status; (4) Notify providers performing cardiac catheterization or imaging-guided interventions (codes 33882, 33883, 33886) that radiologic guidance/supervision/interpretation may now be billed separately when medically necessary and properly documented; (5) Ensure all denial letters reference MA00.019k when radiologic guidance claims using deleted codes are rejected. Claims submitted after 01/01/2026 using deleted codes will be denied; claims using new codes submitted before system updates will also be denied.
